What is the actual bonus?
On Nightmare and Hell, monsters get a damage & AR bonus is 6.25% per extra player above 1.

For example, to determine a monsters damage, you would take their monstats.txt value, and multiply it by their monlvl.txt bonus, then divide the result by 100:

Monster Min Damage = (Monstats.Min * Monlvl.Damage) / 100
Monster Max Damage = (Monstats.Max * Monlvl.Damage) / 100

To include the extra player bonus, you could simply modify the above equation to be:

Monster Min Damage = ((Monstats.Min * Monlvl.Damage)/100) * (1 + ((PlayerCount - 1) * 0.0625))
Monster Min Damage = ((Monstats.Max * Monlvl.Damage)/100) * (1 + ((PlayerCount - 1) * 0.0625))

So effectively, the increase in damage is:

players 1 = 00.00%
players 2 = 06.25%
players 3 = 12.50%
players 4 = 18.75%
players 5 = 25.00%
players 6 = 31.25%
players 7 = 37.50%
players 8 = 43.75%


When is this damage bonus applied?
It's applied on monster spawn. It appears to only be applied to monster attacks listed in monstats, and does not transfer to monster skills. Both Ranged attacks and Melee attacks receive the same bonus. This bonus in damage is identical on Nightmare and Hell.

Note too that this bonus is seperate to other damage bonuses such as those provided by champion

So, for example, if we're talking about a champion monster, we're effectively looking at:

MonsterMinDamage * ChampionBonus * PlayerBonus
MonsterMaxDamage * ChampionBonus * PlayerBonus

Finally, this damage is calculated before the 5% critical bonus that the monsters have.


What actually gets amplified by this damage bonus?
Monster skills do not get increased by this bonus. So, Meph on NM and Hell would get this bonus to his melee attacks, but not his meph missile, lightning, poison nova, frost nova, etc. Likewise, a fallen shaman would not get this bonus to his shaman fireball.

That said, the bonus is applied to the tainted's lightning orb, and lightning damage applied to his melee attack. This is what leads me to believe that the damage is applied to damage sourced from monstat, but not from missiles.txt (which is where the shaman and meph get their elemental missile damages).

I'm going to go out on a limb and say that whatever is increased by the monlvl bonus is probably also increased by the player bonus. That is to say, the shaman fireball for example isn't affected by the monlvl bonus as it's damage is directly calculated from the missiles.txt. Therefore, the shaman fireball is not affected by the player bonus.


Does this also apply to Classic Diablo?
No, Classic Diablo monsters do not gain this bonus.

The number of players in game will affect the %CB accordingly.
For example, a monster with 450% more life due to a full 8 player game will receive a successful Crushing Blow enhanced attack with a reduction equal to its New-Valued total health. The New-Valued life would be (Assuming X=100% monster life):  X+(X*4.5) = New-Valued X. Hence, the reduction will be the %CB devided by 4.5, in this specific case.

-100% CB is attainable through all slots combined except dual-wield (refer to the same explanation presented in "Stacking Deadly Strike").
-Only positive physical resistance is applied when dealing %CB to the monster, if it has -% physical resistance, it will be considered 0% physical resistance. Anything lower will be ignored, regarding %CB final damage.
-1.10 patch brought with him a difference in calculation of %CB, in which up until today we calculate Crushing Blow before the character's normal damage. This means a successful Crushing Blow attack would first reduce the monster's percentage of life, and only then will the remaining monster's health be reduced further by the normal damage applied with that same exact attack. To make this easier to swallow: Attack Monster --> Successful --> %45 (for example) CB applied --> New-Lowered HP --> Normal attack's damage is now applied.
-%CB probabilities stack, and are not checked each at a time, but as a whole random check.

FAQ

1. What is the Dice Roll Effect?
Answer: A simple description of chance. Chance is a factor one cannot predict. Even with a 99% chance to inflict "A" circumstance, there can be a scenario in which in all 10 subsequent cases, "B" factor will be randomly chosen. Random factor cannot be affected by any increment in chance or otherwise, although it does help the occurrence to apply "more often", though it's an illusion we keep feeding ourselves.
A dice having rolled "6" 100 times in a row doesn't have a higher chance of rolling "6" at the 101th time, since the sequence was individual and random to that specific roll, from which one could learn absolutely nothing of the behavior of numbers.

2. Why not post some in-game results to gain a believable feed of the game?
Answer: Refer to the above. One could learn nothing from 1, 10 or even 100 in game tests, since they all run on randomly generated numbers. Since "random" has no true predictable behavior, it cannot be relied upon. This is also why %MF, another in game random-reliant factor, could never be tagged as a good amount at X integer. Nobody knows what a good MF amount is since it simply doesn't matter. One could be lucky and land a 10,000 FG worth rare in several runs using a 50% MF character, while another could spend years with a 700% MF character and never find such an item.
Since the only way to give a technical example of it is bordering the maximal and minimal ranges of those amounts, one could only look at the average of the two extreme measurements and by doing so meet them "half-way". Though it could be that the average will never be applied, it gives a good "aspect" of what could be.
Using that and WCS I was able to provide with a technical, though seemingly unreal, approach of the situation, which some may not agree with.

3. WCS???
Answer: I call it the Worst Case Scenario. I took every percentile of the Crushing Blow effect and applied it only the end of the borderline. Meaning, when there is a 40% chance to CB, I applied it to the last 4 strikes of a 10 strike sequence. True, this is "unreal", but since in game tests cannot be relied on, this gives a scenario in which chance acts as a tangible concept which could be handled with. Applying it to the last strikes will result in the least damage given- while taking into account that it has 40% CB and not giving it less strikes- is the most accessible way to receive on-paper results. True, even with 40% CB there may even be no successful CB strikes at all or contrary- apply to every strike in the sequence, but that is why chance is chance.

4. What is all this topic telling me? ISO tl;dr version
Answer: Treachery is simply better than Fortitude. In ~80% of all cases when raw damage is applied, and better in ALL cases in which CB is applied. In the eventuality of killing a mob, Treachery kills faster. It also makes for a better shell for the mercenary to be a stronger tank.

5. I see Fortitude has a lot more gfg defense, like +30% less chance to be hit. Why isn't that better?
Answer: Well, although this is true, and although receiving less strikes means less likelihood to be interrupted / being thrown into FHR animations, this is only true regarding melee physical attacking / ranged physical attacking mobs. A quick look at an online bestiary will show you that the greatest majority of the game mobs throughout the acts constitute of magic/elemental damage mobs, and even those who melee attack have, for the most part, a magical/elemental attack as well. Knowing this, and taking life stealing into account, this factor is simply not worth relying on.

6. Will you do more comparisons for me if I gave you other setups pl0x?
Answer: No. Use your brain. This is 3rd grade math, at best.
