They also film it in a way where these lower people don't have a single clue what's going on. Not like they see the script, or are around when people are delivering their lines. At best, they might see a location without the added CGI, which you're not going to be able to ascertain much from (most the time). As Blah said, NDAs are a big deal as well. The extras being used in sensitive scenes, are most likely people who have proven their trustworthiness throughout the series, or close friends/family of people in high positions on set.

This is also another factor. They've been absurdly secretive with how they've handled everything this season and I'm positive that there's not a single rando who was actually present for an epic/spoiler worthy scene.
