There were 500.000 players in first D2R ladder, so 1 in 3 bilion would be 1 in 6000 drop.
Combine it with 2 years and thousands of botters, and it becomes quite possible to drop.
You still have 1 missing mod in what you wrote above (5/6 mods).
Eth rep ~270+ED 3-4 skill 40ias Jav is 1 in 3.000-5.000 billions odd, and there were several of these on JSP.

Ive seen two of these on D2R JSP, but im 99,99% sure these were fake
