Actual Formula:
25(Amount of Life Replenish) / 256 = Life Healed/Second
Example of the Formula:
25(5) / 256 = 0.49
(Actual Calculated Number: 0.48828125)II. The Reason for Replenish Life:

Assassin: Totally recommended on trap. Trappers give up very little to reach the 40 cap. Furthermore they have the ability to avoid attacks and apply pressure on offense and defense which allows you to regain as much life as possible. When push comes to shove, defensive trappers (note there is a world of difference between good and bad defensive sins) can rep quite a bit of life while still retaining the ability to deal damage at a distance. Offense is also rewarded. Rep works no matter what you are doing, and an opponent being attacked by a good trapper has less choices in which to attack himself. Ghosts/hybrids gain less from life rep, as they can't get it on claws/amulet/one ring and have much weaker defensive abilities compared to pure trap.
Amazon: Don't bother on bow. You could make a specialized rep build but it greatly decreases your damage and would only be good vs trappers anyway. Plus they can conserve mana much better than you (and don't have ammo), getting into a rep war with a sin is very dumb.
Barbarian: Don't bother. The only application for rep is vs a trapper 1v1, where you could gain significant rep with one hoto and rep on rings (use a strength wc wep for strength bug enigma here). Abusing rep vs a trap here isn't a bad idea, as you do so much damage to her with a good whirl, and if you are good at wsg, you can get in and out of traps fairly quickly.
Druid: Druid is one of the most versatile characters when it comes to rep due to the fact you can manipulate your oak. In a similar fashion to repping without bo, you will gain more effective life by repping when your oak is unsummoned. Just be aware that attacks that kill your oak first will not take into account this "extra rep" as your oak is down. Oak rep is most effective vs attacks that target you without hitting oak (barb whirl if you are lucky, trap mb, etc). Also if you are hit with open wounds, keep your oak up, as OW is static damage, and will take less life in terms of % then if your oak was down.
Necromancer: Life rep is nice on nec, but is only essential in one duel, versus trapper (see a pattern here?). If the trapper is c/ting traps from off screen and generally playing a good defensive game, your only way to counter this is with high life rep. If you are very rich, you can get 30+ life rep with white, which is an ideal setup.
Sorceress: Don't bother with rep if you are vita. Vita sorcs are only good svs, and you'd have to rep a LONG time to be able to tank another hit. Worth it for srs pride I guess ~_~ If you are es get as close to 40 as possible. Nuff said.
Paladin: Not worth it on v/t smite, etc just medi if it's pride. Hammerdins main hoto/dungo, so that's a good chunk of rep right there. Vs trappers, 20 rep from hoto and medi is quite sufficient (overpowered really).
All classes: Repping without bo will grant you an increase in life once you bo again (say you rep 100 life, that 100 life is multiplied by bo granting you more total life points). This technique is very useful in pride duels, just don't get caught without your bo ~_~ You can combine this with oak rep for some insane gains ^^
