I don't care much for the SAME weight each set - I prefer rest and progression or exhaustion with digression.
5x5 in my eyes should be ascending from 5x lower/medium weight all the way up to 5x of absolutely traumatic reps.
For hypertrophy, you NEED to fail...period. You have to force your muscles to fail, then force them to work some more. You have to dig deep and do unholy things to your body.
So if you want me to revamp the whole thing, I would just put you on a different plan all together.
With regard to the actual exercises themselves...you're fine. Just add some more into accessories (triceps/biceps/calves/abs/traps)
