To somehow get gear from my playstation account onto a switch account?

I assume you are talking about d2r.

Yes, it is possible. You need a valid bnet account for both consoles.
If you don't have that, you need a friend who can transfer it for you. He will then also need an account for both consoles.

quite a few people have both platforms, they can grab the item for you on Xbox and log in on switch to pass the item to you.
