I should add
one of my recollections from the werebear / werewolf "missing attack" bug from LoD was that server and clients calculated attack speeds at different rates at high speeds
its likely that D2r took the server calculations and made them apply to client, or vice versa, and thereby fucked up one of the two in a way it wasn't before
it used to be that if your werebear attacked faster than the server thought he could attack, he'd keep missing attacks
from a basin 2011 post;

The problem lies in the difference between the attack lengths calculated by the server and the client. Both Hunger and Rabies suffer from this bug, maybe because they use the same animation mode (S3). For most feral attacks, the server calculates a much shorter attack length than the client, but for these two skills, under some gear and skill IAS settings, the server's length may be the same as the client's. Whenever this happens, every other attack will fail to start on the server, but the client merrily plays the animation for it anyway.
