Have you been to Japan? They were fully masked up before COVID. They have a serious 'sniffle' social behavior, rather than touch their nose. They will sniffle on for hours. The average Japanese makes an American look like a dog rolling in a muddy puddle.
Also, their spiritual places are outside, socially distant.
That is one factor for sure, but even more remarkable is their total deaths.
They probably have 10x as many cases as officially reported, but people are just more adapt and strong when they aren't fast food addicts under a government that subsidizes grain and corn syrup.
