there's no such thing, and this is a great example: people can just call each other with different religious/supernatural or otherwise unreasonable beliefs "mentally ill" and nobody would actually be describing anything physically wrong with someone's mind
Every day there are real doctors & scientists who devote their lives to addressing mental illness.
Mental illness was first understood by the role that syphilis bacteria had on degenerating the brain & thus producing abnormal behavior.
This established the organic view of mental illness, it set precedence for physical causation. Science now treats it by understanding the chemical imbalances.

Mental illness, low iq, too young to have concrete thinking & or the inability to distinguish reality from fantasy are all possible explanations.
