Accidental fires happen all the time in Naval incidents. One infamous case for the US military being the USS Forrestal, which killed dozens of sailors and was caused by a Zuni rocket that misfired (supposedly it was cooked off because John McCain was fucking around with his afterburners on the deck.)
And then once during the Iran-Iraq war an Iraqi Exocet missile fired on an (at the time) friendly US warship and killed 20 people.
Point is, these things can happen easily but I wouldn't put much stock into Ukraine's claim that not only are they responsible for hitting the ship but that they managed to sink it. Seems like they saw what happened and immediately took advantage for a quick propaganda win.

The Stark's countermeasure systems all failed to detect the incoming missile, so you can't - in your extreme state of ignorance of naval warfare - make such a ridiculous claim. Fire is always a concern, as is weapons maintenance (see the Kursk incident). John McCain was in the aircraft struck by the Zuni rocket, you clown.
