Spare me the sanctimonious outrage. I expect better than for you to uncritically accept whatever the prevailing narrative is on MSNBC. Questions about a president's mental fitness go back to Reagan. We spent the last few years listening to high ranking members of Congress and large tracts of the media asserting that the President was mentally unfit. It's a little late to pretend that this is beyond the pale. And because of how important and mentally demanding the job is, we should be asking these questions explicitly.
He regularly forgets names and what he was about to say. His aides kept him away from a live press conference for a record amount off time (

Compare this to Clinton, Trump, Bush, or Obama and there isn't any comparison. He is declining mentally. I have a little more respect for the office than Nancy Pelosi, so I don't think it's time to say that we should invoke the 25th, but it's certainly something we should be concerned about.
