Covid risk only ramps up in age-groups 60+, and only reaches really high levels at 70+. African countries which have almost no residents at this age will obviously see far less deaths than the Western world with its geriatric population.
Based on those numbers that isn't the case. The % death is almost the same between the two groups, so the reason their numbers stayed so low is because it never reached a level of community spread, which is mostly dependent on international travel and population density.
