Found this little gem from a (lvl 69) treasure chest just now on HC. No screenshot available.

Nothing special but clearly higher than a tier 1 or tier 4 item.

Rare wild heal XI
Level Req: 40
Heals: 26 to 264
Mana Cost: 85

+91% Enhanced Effect
+6 Critical Strike

Uber chest at the end of a tier I patched map with 25 lvl cap -- 3 players in-game :)
