I played D2 PvP for about 10 years and that is in my (and many other's) opinion the real endgame. It has a very steep learning curve and knowing the bugged and glitched game mechanics is part of "getting gud". When you are at a point where everybody is using the same thing, then it is up to the skill difference again to decide who is better.
Running baal and CS for thousands of times is not really endgame content but battling it out with others has kept the game (and economy) alive since the buyers of good items aren't the baal runners but the PvPers :)
Nobody said it is easy and D2 is a very unforgiving game with very little hand holding and should stay that way.
but only a small % of ppl really get into pvp, and even a smaller % becoming good, and i don't know why many don't even give it a try (maybe the bms?) (get a decent gear for pvp is to expensive?)
to this day i belive that d2 have the best pvp experience that i ever play and i dont found on another game, not even close.
