disk.sys can be a few things buts its generally failing ram/disk/broken ntfs file system.
I'd personally fresh install windows and manually update all drivers. fastest way to rule out all software.
After that I'd Test Ram/HD with memtestx86/hdtune if your issue continues.
With HDtune post a bench mark and a deep scan result. with memtext x86 you want 7x passes minimum.
errors in either means replace the item... you may get nor errors in hdtune but you need to post benchmark to be reviewed.
All the other errors are super generic and could be numerous things so I'm focused on the disk.exe one.
Most share the same advise anyways....
Now that you mention it, the second SSD I use for all my games sometimes isn't recognized, then after a restart it is.
I can easily switch it out and re-download my games as it's my gaming HD and not my main.
Also makes sense because it usually crashes while playing SC2 and I ran memtesk and had zero errors. And my nvidia are up to date.

how full is your drive?
It's a 512 about 3/4 full
