A lot of game engines actually assign numerical values to drops to simplify the randomization process. Runescape is a common example where drops will be assigned a range based on their weighting, as confirmed by several of the game developers. Considering the game is 20 years old, it wouldn't be surprising to know they used oddly arbitrary/obfuscated methods of randomization. Could also be a quirk with the game engine. I know nearly nothing about modern coding, let alone 20 year old game engines, so I can't say with any certainty, I just remember reading a longer post where someone stated that because there are around 6 billion permutations of rolls for eth tombreaver (not sure if this number counts the 1/12 for ethereal or not) a large number of rolls cannot organically spawn. Maybe someone else who knows more about the itemization/drops can clear up if there's a misconception in this, I am definitely not an authoritative figure.
