In Diablo II v1.09, "Perf Storm" refers to a perfect defense roll on a Stormshield Monarch (base shield defense \(148\) up to \(151\) in the 1.10+ patches, but capped at exactly \(148\) base defense in v1.09), offering \(151\) total defense when including the standard modifiers.

Before 1.09 the item Stormshield was an Aegis Shield. When they changed the base type to Monarch, they forgot to change the base defense, so it would stay 161 (max base def).
When they corrected their mistake, our today's Stormshield came out with base def 148.
Google for a Perf 1.09 Stormshield and you will see, the perf total def is 164. Your Stormshield here is missing 1 defense from total perf. Still a rare and expensive beauty of course but not total perf.
