The closest I could get from a Tyrael was when I was heading to Pit from A1 in a TZ game and I killed an archer (regular monster) and she dropped an unique SA, but unfortunately she was naughty and gave me the green fella

pretty sure it was a white ghoul. Hard to keep track sometimes when farming and watching tv/stream on other screen.
