Take the waypoint from town and it doesn’t warp you to the WP in the area you warped to but rather a random seeming spot on the same map.

Has anyone ever had this?

When it happens does the map spawn with a WP somewhere else or no WP at all?

Whenever it happens the wp is usually missing

There’s been a lot of weird things that have happened since the release of d2r

Most don’t happen anymore though
