1. Force Windows to use your RTX 5070 Ti
This is the most likely culprit. We need to tell Windows explicitly not to use power-saving graphics for D2R.
• Right-click your desktop and select Display settings.
• Scroll down and click on Graphics.
• Under the "Custom options for apps" list, look for Diablo II Resurrected (D2R.exe).
• If it's not there: Click Browse and navigate to your D2R installation folder to add D2R.exe.
• Click on the game in the list, select Options, and change it from "Let Windows decide" to High performance (it should list your RTX 5070 Ti here). Click Save.

2. Wipe the Corrupted Game Settings
Since you mentioned the video settings felt "totally off" and the intro cinematic is black, your settings file is likely holding onto bad data from your first install.
• Open your File Explorer and go to: C:\Users\[YourUsername]\Saved Games\Diablo II Resurrected
• Find the Settings.json file and delete it. (Don't worry, the game will create a fresh, default one the next time you launch).

3. Disable Overlays (Temporary Test)
Overlays are notorious for causing that "unexpected issue" crash in Blizzard games, especially on fresh Windows installs.
• Xbox Game Bar: Go to Windows Settings > Gaming > Game Bar and turn it off.
• GeForce Experience/NVIDIA App: Open the app, go to Settings, and turn off the "In-Game Overlay."
• Discord: If you have it running, turn off the in-game overlay in Discord's settings.

4. Bypass the Intro Cinematics
The black screen at the "Press any key" prompt often happens because the game is struggling to render the Blizzard intro videos. We can skip them entirely.
• Open the Battle.net launcher.
• Click the gear icon next to the "Play" button for D2R and select Game Settings.
• Check the box for Additional command line arguments.
• Type in -skiptobnet (this skips the intro videos and takes you straight to the login).

Try doing Steps 1 and 2 first, as they are the most critical, and then boot the game.

Hey mate, just tried all these steps but hasn't solved the issue

it's very weird, act 5 is like a storm of big lines on my screen
Act 1 grass changes colour each time I load it LOL

open nvidia app, update latest drivers and run diablo 2 from there

if the step above didnt help try scan and repair from battle net launcher. in case it did help you may have to configure from nvidia control panel diablo 2 to run with graphics nvidia instead of default one.

I had these weird visual effects aswell when I installed the game. The solution I found was pretty weird. First I had to uninstall the game. Then install the PTR version, launch it, make a character and go in a normal game. After that, quit game and install normal version of Rotw. That worked. Wow.

Will update.

D2R can behave weirdly with DirectX 12 on newer cards.

Try forcing DirectX 11:
Open Battle.net
Go to D2R settings → Game Settings
Check “Additional command line arguments”
Add:
-d3d11

Edit: also could be selecting the onboard card rather than the RTX

confirming the gaming is running on the RTX 5070Ti
