1. Force Windows to use your RTX 5070 Ti
This is the most likely culprit. We need to tell Windows explicitly not to use power-saving graphics for D2R.
• Right-click your desktop and select Display settings.
• Scroll down and click on Graphics.
• Under the "Custom options for apps" list, look for Diablo II Resurrected (D2R.exe).
• If it's not there: Click Browse and navigate to your D2R installation folder to add D2R.exe.
• Click on the game in the list, select Options, and change it from "Let Windows decide" to High performance (it should list your RTX 5070 Ti here). Click Save.

2. Wipe the Corrupted Game Settings
Since you mentioned the video settings felt "totally off" and the intro cinematic is black, your settings file is likely holding onto bad data from your first install.
• Open your File Explorer and go to: C:\Users\[YourUsername]\Saved Games\Diablo II Resurrected

Disable DLSS completely
If you can get into video settings at all, set:

Upscaling = Off
DLSS = Off
disable any frame generation / sharpening tied to it

Reset D2R graphics settings
Delete or rename the D2R local settings folder so the game rebuilds it cleanly.

Remove Battle.net launch arguments
In Battle.net → D2R → Game Settings, make sure Additional command line arguments is empty.
Especially remove -direct -txt if present.

Force a full asset refresh

close Battle.net fully
go to the D2R install folder
delete the Data folder
reopen Battle.net
run Scan and Repair


D2R can behave weirdly with DirectX 12 on newer cards.

Try forcing DirectX 11:
Open Battle.net
Go to D2R settings → Game Settings
Check “Additional command line arguments”
Add:
-d3d11

Edit: also could be selecting the onboard card rather than the RTX

had similar nonsense occur, how I fixed it :
- go to Bnet launcher and instead of Play, hit the cog button next to it, select "Show in Explorer"
- delete only the Data subfolder & nothing else
- go back to Bnet launcher, same cog button, select "Scan & repair"
- this will re-download most of the data yes, patience
- no idea why this worked when a full re-install did not, makes no sense at all, but it did work
