Not guaranteed but definitely the most consistent way of getting mythics! You'll start getting plenty of these keys if you are killing world bosses on CD with your Divine Gift of Sin at least 4/5 so get your Azmodan farm in if you haven't yet!

Depending on the day and how many I'm running, I'm at between 25-40% drop rate.
