Normally:

Act 2 Might Aura Merc
Reapers Toll
Fortitude (or the Fade Runeword armor)
Arreats Face, with a Ral or Jewel to counter Fire Resistance (just my personal opinion)
... Reach the IAS merc BP with Thresher weapon (Reapers)
... ensure decent Life Leech (Steal) ... hence Arreats Face etc.

This is D2LoD information, however the game mechanics hasn't change that much for D2r.

1.09 Mercs would be Holy Freeze Act 2 Merc with Reapers Toll, Arreats and Shaftstop for armor... maybe the helm was different, but just look items up.

Defiance/prayer infinity coh FF/cure

Defiance / Insight with a Blizz Sorc that can use Glacial Spike to Freeze PACKS of Monsters not Immune to cold?

MaeK to replenish mana? Instead of Insight? or are you spam tele'ing while Blizzard cool down period expires?

Hey, all in all, if you spam tele and run out of mana... yup use insight, it is good at earlier levels I agree, but people become reliant on it too often and don't know how to use strategy and commonsense to bypass the issue.
