Can you please explain quickly how to spot failed set and failed uniques!?
I know it has to do something with the durability... but how do you knows if it should have been set or unique?

Absolutely.

A failed unique has 3x the durability as the normal item.
Looking at a colossus girdle (which does not have a unique version), the durability normally would be 24: https://classic.battle.net/diablo2exp/items/elite/belts.shtml

A failed set has 2x the durability as the normal item.

This gets a little weird or hard to spot with ethereal failed unique/set...

An ethereal item has half the durability +1 as the normal item.
So, a normal ethereal colossus girdle should have 13 durability, but a failed unique ethereal colossus girdle would have 3x the half durability +1 or (24/2 * 3 + 1) = 37: https://forums.d2jsp.org/topic.php?t=89361061&f=161&p=654184263

Don't forget that failed unique is always a rare item and failed set is always a magical item. Iirc bugged andy can't drop items less than rare. All the magic you see is failed sets
