You have to understand, America is all about capitalism, the notion is, if you are capable enough, you can make wealth and buy your own health insurance. Conversely there is no safety net and the US frowns on this notion. A not for profit health care system does not fit in the US model. This is why the US does not want mechanisms that will reduce individuals net worth as they climb the pyramid. To express this into layman's terms, the US is opposed to community support, that is to say, mechanisms to support the community and increase the standard of living of the poorest people. "Why should I pay anything in tax for the man down the street just because i am more wealthy then him".
