is it ok to assume that high ilvl correlates with higher lvl req?

just identified a bunch of supposedly hell-found items and most of them had super low lvl req (lvl30), however I am unable to check ilvl...

iirc ilvl determines what affixes the item can spawn with (affix lvl <= ilvl), and lvl req is 75% (rounded up) of highest affix level on the item

That's not the whole story. Every monster has a list of what they can drop. The item drop and affix generation system in d2 is unfathomable.

ilvl corresponds to the level of monster you killed in a given area of hell. For example, andy is 75, meph is 87, diablo is 94, nil is 95, baal is 99. Baal can drop ilvl99 bloodfists, has nothing to do with the level req of the item. You just had a coincidence of the affix/suffix combinations rolled low so the level reqs rolled low. The two are not correlated as mentioned previously.

No, mf just influences quality of the item. I.e. unique, set, rare...
The only minor way to influence affixes is to farm ilvl areas where your highest desired affix can spawn. So item generation has less affixes to choose from.
