Only thing I see that seems odd (besides the death) is your spirit became partially unreserved (jumps to 65/160), so possibly some very weird thing happened on a gear swap?

if you lagged, might've been the water mobs?
there's a swirly water ball attack that just causes instant death if you stay in it for a couple seconds. the tickrate might scale with action speed, I'm not sure.
