Basically I did half of campaign on hardcore by myself then did other half in co-op.
Last weekend I did insane mode with a friend in co-op and the achievement still hasn't unlocked???

Anyone know what I am supposed to do?

just go from the very begining.
it only makes it complicated when you don't.
and im pretty sure you hvae to train carmine.
