Stacked resistances, a fast two handed weapon like etomb or even botd great poleaxe, and lots of practicing the shift key and keeping your opponent at the edge of range is really all I've got. Don't be afraid to run away to get range advantages back. And keep werewolf on right click and get good at reshifitng. Especially useful vs hammerdins and chargers.

Trangs + Valk + arach + fcr ammu + 10fcr rings for 99fcr eq
Duress + Valk = 48fhr break, although fhr is not uber important on this build.
