Hmm you say you only have 1 point in telekinesis?
So that means that for every 1 dmg you take, you lose about 2mana (actually 1.9 or something) and 0.25 health(with 75%energy shield). With good resi and dmg reduced, this dmg to life is even further reduced.With only 3.5k mana and 1 point in telekinesis, 1-2 hits kill your mana completely and you are unable to escape because no mana left for teleport. I think it would be wise to either:
1)put more points in energy instead of vita
2)sacrifice some points in a synergy for lvl 18telekinesis (on this lvl you lose 1mana for every 1 dmg you take)
(Keep in mind that only ACTUAL points in telekinesis help your energy shield, not +skills from gear)

Ah I misread it srry, under skills it says: 1tele obviously, I interpreted this as 1 in telekinesis

how do you bug tals arm?

its an item from 09, which as far as i know is only on east.
