i did not manually level to 99 in single player, i use single player for testing i was just explaining the differences in how if you wanted to level to 99 in single player it would be faster and safer vs online.

i have only leveled to 99 while on closed uswest, there is even a link in my signature of me doing it.

Only happens if in a full group, you leave the group just as you are about to kill off the Main boss (Diablo/Baal) in Hell difficulty... there are a few builds that can do this.

Light Javazon / Berserker Barb and Hammerdin.

So, re-evaluate the question by character class.
