I would not recommend using a CoA at all, A viso dia would be best, or perhaps a DS viso armet but if you must use CoA then try a crafted OW belt- something like high defense 10OW 24FHR 30str etc
I would not use shaft in a NT setup, if you don't like fort, try prudence and see if you like that
don't forget that w/o a raven you'll need to incorporate a cham rune in a socket

If they have a source of cold dmg (like from a ravenfrost) you'll freeze which is why the cham is still necessary.

Imo fhr isn't something to worry about in gm melee. Since zeal is uninterruptible fhr only comes into play between zeal volleys which is why even going with 0 fhr is fine. Shako setup doesn't hit any fhr bps and doesn't have a problem most of the time.
And there aren't enough posts yet imo
