LoH are bis for any pvm phys bowa.
Imho you don't need kb in pvm.

Usually that what you got from ring is enough (if you don't spam Multi like maniac).

Kb works in pvm but imho doesn't make a relevant difference.
I would choose that 350% vs demons all day long.

You should try telebowa, especially hybrid. You will get way more mobility and you will kill bosses a lot faster.
