/e the only context of ds/cs stacking is that it doesn't quadruple your damage. I saw nothing to say it does not work together to give you 100% double dmg if you have that much cs/ds combined. And that IS from the official d2 website. Not what's written above, unless I missed it somewhere.

(1) We have 4 options: scoring a CS, scoring a DS, scoring both, or dealing normal damage (neither CS nor DS).
(2) Since we cannot score both (quadruple damage), the case 'CSDS' must be discounted (subtracted) from (1).
Scoring both CS and DS (independent checks) would amount to a chance of
CS%*DS%/100.
In words, we either score a CS (odds: CS%) or we fail the CS and score a DS (odds: (100 - CS%)*(DS%/100)).
Since they are clearly disjoint, the final chance is obtained by adding probabilities.
DD% = CS% + DS% - CS%*DS%/100 = CS% + (100 - CS%)*(DS%/100)The order in which the game checks for CS and DS has no bearing on the result (the expression for DD% is symmetric with respect to CS%, DS%).
Because CS% cannot reach 100 in practice, double damage is only guaranteed with >= 100% Deadly Strike.
Anything above 100% is discarded.
