Scripted randomness is not true randomness. But it is still highly randomnized.
It has been exploited on singleplayer, where you can use the same map seed over and over again, and even choose map seed.
Stuff that affects it that i know about is:
Number of summons
Numbers of items in inventory/stash/cube+++++++
What areas you have visited. Which parts of the areas you have triggered.
I think time may be a factor as well. But not as much, i think there are tresholds.
Playercount affects it as well.
People, including me has been able to drop the same excact item with the same excact stat over and over again on singleplayer.
Dropping the same item base has less factors. Only map seed and visited areas(that session). So stacking up loads of runes of a certain type is quite easy.
However, the stats inside the item changes with summons/items in inventory. So as soon as you have picked up 1, ur number of items in inventory is changed.
