how is holy shield unspawnable? I have a +3 holy shield cta

IIRC
It attempts to roll a valid skill, and if it chooses an invalid skill (like holy shield, for example) it rerolls.
If it fails a reroll 3-4 times it simply goes with whatever to prevent the game from crashing.

Combat skills (barbarians only) are associated with weapons. They are not supposed to spawn on primal helms.
Poison dagger is associated with daggers, not supposed to spawn on wands or shrunken heads.
Holy shield and smite are associated with shields, not supposed to spawn on scepters.

This is a feature of 1.08 (IIRC).

Now, due to an internal process, it is however possible to spawn all those skills on forbidden items, but with (very) low chances.

this, rerolls 7 time then accepts last skill.
