I dont think I can tell you the gear for it since I haven't tried it myself.. But I can't see why it wouldn't work. As long you make sure you have a nice dmg, put some draculs on it.
Make sure you have enough stacked res (which is going to be a bit hard I think on shape druid, maybe with sc's).
For ubers I would'nt consider bear because of its ias, wolf it is. I think you just have to figure your gear out while trying.

All you need is high IAS, Crushing Blow and Life Tap or a leeching Skill to survive...

Guillaume's, Goblin Toe and some CB from your (fast) weapon, and your done basically.

lose the bk for another raven and maybe use angelics if you find the ar too low. as for the armor, coh seems to be the better choice.

a good weapon would be gris caddy 3x shael + ber or like a beast zerker, but I'm not too sure about that. then there's the option to go shieldless, which I find more viable. Use an upped ribcracker for that. eth or non-eth, doesn't really matter.

Consider the belt twice, sure it does give you vita and drain, but TG gives you massive abs vs mephi.
Stormshield is the easy way, I don't see mb being important for ubers, Phoenix gives you +max res and maybe a usefull aura.
Coh to give you the res needed for your druid. The CB and open wounds is enough for ubers. The rest of it is enough to kick their butts.
(Maybe some random CB weapon on your 1st slot? no idea)

Upped ribcracker should do the work!
