That is because they don't have difficult content released so far, even Valtan hard mode, where every drop of DPS contribution counts.

20% total damage is superior to 16% attack damage.

atk pwr =/= more dmg from grudge

Ive found more gear with cursed doll than grudge so i was able to use AH to grab a few more to get the 4x3. I've also heard the importance of staying alive as opposed to a little more damage output in valtan but i could be wrong. Is it a huge difference to run grudge over cursed doll? More than happy to take any input/suggestions. Maybe ill change it to grudge after some valtans.

Eventually you'll have to run both grudge and cursed doll, your objective after Valtan with the relic accessories is to make that 3 x 5 happen or even better 3 x 5 and a level 1 or 2 class (Mayhem for you in that case)
Your best bet for staying alive is to actually learn mechanics and attack patterns, since Berserkers have absolutely no fear of the extra damage taken from Grudge because Mayhem plus your cards will provide enough damage reduction already.
