Your cc seems a lil bit low.Roll amulet 20 % fire dmg -> cc . Also Ik helm, roll Hota dmg % to 6 cc
Chance rings for focus and restrain (Furios Charge generator, HOTA spender so almost always +100% more dmg)
If u need unity pick up it from cube and if not pick up CoE.
ur passives...all defensive..u need dmg to kill mobs faster that they don't kill you.
Rampage - ruthless - nerves of steel - berserker rage + amulet brawler are good choices.
If u need more survival, roll ik belts life% to life per fury spent. Roll ik armors reduce elite dmg% to 15% life (this depends, but ur life is low so i will do that). Try to get Gavel of judgement with vitality or life per fury spent or with both.
If u using life per fury spent, ur resorce cost reduction should be 0%.
