I would be glad if you could tell me on what improvements I should concentrate myself on.

I just did Grift 42 and the speed while the Grift is good. But the boss takes me nearly the half of the timer so it gets hard to progress there.

Personally I would swap out gogok for Pain Enhancer. Allows more damage. Could also swap out reapers and swap in Weapon master passive. Opens up options in bracer slot.

And on the item-side? My dmg to single target ist just insane low...

You want gorok, both for damage and survival.
