Anyone any tips on where to find one? I've only ever found one and unfortunately it wasn't ancient :(

Speed greater rifts at levels 35+ or just 4-man T6 rifts.

So you're saying you've played long enough to be capable of achieving 40+ grifts, yet you don't know the most basic information about the game? How is that possible? You must have gotten lucky as hell to stumble your way to where you're at and to still not know that it's completely random for that item to drop or be gambled.

Prior to creating this thread you never had two of the same item drop in different locations, such as playing a bounty, playing a rift, playing a greater rift, gambling from kadala, let alone on any specific map?

It is very common for all new players to ask your question, but achieving greater rift 40+ isn't a trait new players tend to have, which means you have played for more than a few hours, yet avoided absorbing the answer to your question while playing as much as you have. Learning the answer to your question is a side effect of playing the game, not studying it. What do you think the answer to your question was going to be? Act 2 Sewers drops Serpant Sparker? Do you think Tal's armor drops in greater rift keeps only or only Unity rings can be gambled?

TL;DR - Other than bounty-specific and craftable items, ALL ITEMS RANDOMLY DROP AND ALL GEAR CAN BE RANDOMLY GAMBLED. The only things that aren't obvious are some crafting plans.
