Reflect can be handled VERY easily with just a few things:

LS on weapon (3% would be a very good amount to aim for, LS/OS Manajuma's is not very expensive at all)

Jaunt rune for SW (This will put you in the spirit realm, negating X amount of damage. Jaunt increases the duration of SW)

Increased health from health globe/potions. I try to get one item with ~10k)
Other than this, your skill setup should be survival/kill speed orientated only, with optional focus on CC. Mana regeneration is necessary to account for increased survival of enemies.
With that in mind, this is the build I've come up with which handles MP5 with only 80k unbuffed DPS/30k Life:
http://us.battle.net/d3/en/calculator/witch-doctor#fWUdQY!aeU!cbabbbZB obviously your main damaging spell, with SW for positioning/survival, RoT for Mana (keep VQ buff up 100% uptime while setting up for/casting ZB), SH for buff/snare, BBV for Elites, and Zombie Dogs for additional DR. Passives are survival/resource regeneration orientated.
Rotation is generally something like:
SH @ 5 (SW in if no Reflect present) --> position against wall for range, BBV --> 1x RoT to start VQ --> 3-4x ZB (SW if Reflect)
Maintain 5 SH, 3 Zombie Dogs, VQ
