but because baals soulstone was not destroyed his influence remained on sanctuary and he devised a plan to corrupt tyrael like he had done the worldstone and open up a portal to hell so his brothers and their demon followers could enter the world of sanctuary.
how he did this is still unknown, but the meteor that stuck the worldstone crater: the "harbinger of destruction" was created by baal, and it dislodged the barier enough for his armies to seep through.
as the hero you must find a way to free tyrael of baals taint before the barier is completely destroyed and the prime evils can freely walk the world of sanctuary in their true forms.
