leah is the vessel for the spirit of evils (the black soulstone) that adria uses on her... which makes she becomes "Diablo"... it's not really diablo imo, its a mixture of all demons.. hence the diablo's difference form ... and at the end "diablo" kills the angel Imp. and takes over the heaven and we have to defeat her (yes, the battle happens on the heavens) and once we do theres a cinematic of her body turning to ashes as it falls from the heaven, so to answer your question.. yes, leah dies. :)
