I don't use silencers for one. Knowing the weapon range when taking your shots is also a factor and adding long barrel to things like SMGs/shotguns will help a little. Then there's lag where you end up hitting limbs instead of head/chest/waste.

worst is the ksg - perfectly lined up shot and no hit marker...drives me mad.
