For the past couple of years I've been feeling very tired all the time but I don't know why

I get physicals every year and everything checks out fine

I used to be very active 3-4 years ago lifting weights and running. Nowadays when I try to workout, even if it's just lightly jogging or lifting light dumbells like 10-25lbs I get very sore for days and can't do anything. Sometimes will have to take Tylenol because muscle cramps afterwards. When I try running I'll get short of breath after like a quarter mile... am I just really out of shape? Doesn't seem right. I really want to workout like I used to but feel like shit after doing light activities

For food, I usually try to eat healthy stuff like sandwiches and stay away from fast/junk foods
I also take multivitamin every day

Doc wants to give me depression and anxiety meds to try... webmd says I have cancer/aids/etc lmao
Figured someone here may have some other suggestions or insight

Could be out of shape that's true. It's tough to restart working out so take it easy to start out. What do you currently do besides what you already mentioned? Are you eating enough calories for your weight? How's your water intake? How much sleep do you get per night? How's work or school?

go see a doctor or even a clinical psychologist. Seems you may be having some mental issues, although it could be a physiological condition like you think.

Low energy/feeling like shit/ etc. can easily be symptoms of depression.

you may want to have your bloodwork checked out. you may have a thyroid issue on the hypothyroid end of things. otherwise, you may need special work done to look at your mitochondrial health. it's also worth checking on whether you may have a parasite or an unhealthy gut biome.

if it's feeling tired that's getting you down, stay the f*ck away from anti-depressants, for the love of god. if u feel depressed as well as fatigued, anti-depressants are still not the answer. dietary, sleep and behavioral changes are in order -- that and getting yourself into a better general mental state.

for a temporary boost in general (for energy production) and a good motivation to get back into lifting and seeing results, consider hopping on the creatine train.

if you're experiencing increased soreness with exercise you probably have too much inflammation--that or something's going on with your nerves.

everything checking out fine in a physical doesn't give you a very clear picture of your overall health. if you're experiencing fatigue and pain (physiological symptoms), you have a physiological problem to solve :)
