Sometimes I push myself really hard to get the last rep in and end up feeling really dizzy, light-headed, and pins and needles feeling in my forehead and feel like throwing up.
I usually have to wait like 5-10 minutes to recover for my next set when this happens.

Does this happen to anyone else and is it normal?

That isn't normal.

Its ok to feel a bit light headed/flushed but it shouldnt last that long.

This sounds about right, as I do my workouts after work on an empty stomach. I also eat very few carbs in my diet (like 15-30g per day).

Should I be adding carbs to my diet just for my workouts? And what kind of things can I eat pre-workout?

bananas 20 min before workout (f.e.) on the way to the gym for the win.
Make sure you get enough sleep so you are well rested before workouts.

Drink Gatorade during workout. Pre-workout you can get some simple stuff in like a banana, or drink a shake. I've done pizza slice pre-workout before lol.

Planet Fitness confirmed? :D

labatymo, that sucks. It's not normal, but I think that's just the way you deal with the intensity of that last rep.

Either don't go full-out or try to keep your breathing in order as good as possible.
