Been running a 5x5 the past 2 months. I've seen gains however I had a slight problem yesterday. Last week my last set was 250x5. I did them even though the last rep was tough. Decided to keep the 250 for this week too. Could barely do one rep. I am sufficiently resting each body part. What could be the cause of this? And how could it be prevented?

If you were able to do 250x5 one week, and the following week, 250x1 was almost impossible, then it could be the following:
1. Insufficient rest
2. Insufficient eating
3. Minor injury
4. Pre-Exhaustion
In order to determine what's going on, take ONE week off from doing legs, follow your diet meticulously, making sure to get enough nutrition, and also make sure you get enough sleep.
When you come back to legs, START with squats (if you don't usually do that).
Make sure you rest enough between sets, and keep your mind focused!
If you still experience problems, we will go from there.
