Chest pain + dyspnea on exertion, could be pulmonary (related to lungs) on exertion, but since you said regardless of what you think or do it sounds like it's a cardiac issue most likely, i'd say it's either possibly an arrythmia or some sort of angina.

does the pain radiate/move anywhere or is it confined to your chest?

id go with the arythmia before the angina, pain from angina would in most case go away when not doing anything/resting.

yeah you're absolutely right

i'm trying to rule out angina though with the radiating pain question

Do you get any headaches when this happens? Like severe migraines that feel like your spine/lower back of your brain are going into shock?

that's called orthostatic hypotension, it's physiological and may or may not be related to your other symptoms.
go see a fucking doctor.
tell him you have angina.
you're at risk for a stroke/MI.

i doubt this honestly lol. because i've noticed that unless i think about heart pain i dont get it..
only thing that i dont understand is the little bit of heart discomfort when i think about sex.. otherwise i dont get it.
if something was wrong with my heart i wouldn't have been practicing for 90 minutes of intense basketball training just 1 month ago
