Vaccines are divided into 2 different groups: Live and Killed.
The strain of the virus is killed with formalin/formaldehyde or beta-propiolactone or alcohol. (these ingredients are carcinogenic, but I doubt it's what she had a reaction to).
Live vaccines are more dangerous and have a chance of replication within the host.
Killed vaccines have carcinogens in them and other dangerous substances.
Basically this girl could have had a pre-existing medical condition (allergies, hypersensitivity), or some sort of immunodeficiency/immunocompromised condition that was not mentioned in the article. Which lead to her death. She was only 12 ffs (there is no need to give a 12 y/o girl HPV vaccination).

Indicates she died of cyanosis (blue/purplish discoloration of skin/mucus membranes due to low o2 saturation).
So she died of hypoxia, there was not enough oxygen being delivered to the body.
Causes of cyanosis are based off the different organ systems.
Respiratory - COPD's, emphysema, bronchitis, or anything restricting air flow.
Cardiovascular - basically any sort of heart/valvular failure, stroke/MI.
Blood - any sort of hemoglobin impairment causing decreased o2 carrying capacity.
CNS - seizures/epilepsy
I'm thinking this girl could have had an allergy/hypersensitivity reaction to the allergen/virus present in the vaccine thus causing constriction of bronchioles and airways.
Or the allergen could have caused some sort of immediate heart failure.
What we know for sure is that she died of
CYANOSIS. According to what I inferred from the article.
